Memo: Customer and Visitor Register 

To our valued Customers & Visitors,

To assist in complying with our Business’ COVID Safe Plan, we respectfully ask that you refrain from entering the premise if you:

1. Have presented with flu-like symptoms (or a related illness) in the last 72 hours including but not limited to cough, fever, sore throat, fatigue, loss of sense of taste or smell, or shortness of breath;
2. Have been in contact with someone who tested positive to COVID-19 virus within the past two weeks; 
3. Have been directed to self-isolate for a period which includes the day of visit; or
4. Have been tested for COVID-19 and are awaiting results.

If you do meet any of the above criteria, we ask that you inform Shade Australia Management prior to visiting us so we can make alternative arrangements.

While on premise we do ask that you:

• Complete the register at our front desk;
• Maintain social distancing 
• Practice good personal hygiene; and 
• Download the COVID Safe App.
